No additional doctor. No new equipment. No bigger marketing budget.
Just getting significantly better at what is already happening inside your four walls.
In this week’s episode of Power Hour, host Eugene Shatsman sits down with Jamie Rosin, Co-Author and Creator of Eyecare B.O.S.S., following the first Eyecare B.O.S.S. Live event to unpack one of the biggest discoveries practice owners made in the room:
Many practices are already sitting on hundreds of thousands, or even millions of dollars in unrealized opportunity.
And the opportunity may be hiding inside metrics most owners already track.
What If Growth Doesn’t Require More Patients?
At Eyecare B.O.S.S. Live, more than 200 participating practices documented over 3,000 growth ideas before narrowing those ideas into specific 90-day commitments.
Across roughly $500 million in practice revenue represented in the room, attendees identified approximately $105 million in potential opportunity tied to getting more out of the footprint they already have.
But one of the biggest surprises was how much of that opportunity had nothing to do with adding more patients.
Jamie describes practices realizing they did not necessarily need another location, another doctor, or another piece of equipment. They needed to improve a handful of things they were already doing.
An 8% Improvement Could Be Worth $174,000
Eugene walks through a hypothetical $1 million practice seeing roughly 2,400 patients per year.
Using an industry exam-only rate of approximately 59%, he asks a simple question: What happens if that practice improves exam-only by just eight percentage points?
An 8% improvement across 2,400 patients means just 192 more patients per year make a purchase instead of leaving exam-only.
That’s 16 more patients per month. Or, even simpler, about 4 additional patients per week.
That seemingly small increase in additional patients represents approximately $174,000 in additional annual revenue.
Same patients. Same doctors. Same physical practice. Just better capture and better execution.
And that is only one lever.
What Other Revenue is Waiting to be Unlocked?
Eugene and Jamie then start stacking opportunities.
Dry eye treatment acceptance. Multiple pairs. Premium lenses. Membership programs. Warranties. Contact lenses.
Small improvements in just a few categories can compound quickly.
In Eugene’s example, improving exam-only, increasing dry eye treatment acceptance, and adding a warranty strategy takes a hypothetical $1 million practice to approximately $1.28 million without increasing patient volume.
Almost all practice owners at Eyecare B.O.S.S. Live identified numbers dramatically larger than that.
Jamie recalls attendees discovering potential opportunities measured in the millions of dollars, simply by quantifying what could happen if they improved existing performance incrementally.
The lesson?
Growth doesn’t always require a bigger budget, more equipment, or 50 more patients walking through the door every week.
Sometimes the bigger opportunity is already inside the practice.
It comes from being willing to look critically at your current process, identify where patients are falling through the cracks, and build better systems around the behaviors that drive results.
Done well, better systems can create more predictable growth, stronger profitability, and less dependence on the owner having to constantly push everything forward.
Then Jamie Sees One Number He Can’t Ignore
The conversation shifts to industry performance data.
One metric immediately gets Jamie’s attention: Contact lens capture.
Using data from about a billion dollars’ worth of independent eye care transactions, Eugene has seen that only about 51% of patients make a CLF purchase within 30 days.
Jamie’s reaction? That number should be much higher, because in the multi-location practices he worked with, the benchmark was not 51%. It was as high as 90–92% same-day capture.
Not within 30 days. Same day. And that immediately raises the obvious question: how?
The 90% Contact Lens Capture Playbook
Jamie breaks down the systems that made those numbers possible across multiple locations.
It starts with the doctor handoff. A clear transition that tells the next team member what has been approved and what conversation needs to happen next.
Then the team needs to understand rebates, annual-supply economics, price per box, competitor pricing, and how to respond when a patient says they can buy cheaper somewhere else.
The fit itself matters too.
Instead of automatically handing over trials and hoping the patient eventually returns to buy, the practice builds a process designed to create commitment while still allowing the doctor to adjust the fit when clinically necessary.
The point is not one magic script. It is creating a repeatable system around the sale.
Give Patients a Reason to Buy From You
Then comes another question:
What can your practice offer that an online retailer cannot?
Jamie shares the contact lens service agreement his practices used to add more value to buying directly from the practice.
Depending on the plan, benefits could include future fit discounts, solution, cases, and savings on plano sunwear.
The idea is simple: Stop competing only on the box price. Give patients a reason to prefer buying from you.
Although contact lenses may carry lower margins than eyewear, Jamie and Eugene make the case that these patients can still be extraordinarily valuable.
They purchase repeatedly. They return consistently. And when the practice handles the category well, walking away from contact lens sales can mean voluntarily giving up a meaningful portion of gross-profit dollars.
The Bigger Lesson: Optimize Before You Add
Another theme runs through this entire conversation.
Practice owners love new things. New equipment. New specialties. New marketing. New technology.
But Jamie challenges owners to ask whether the next investment is really the best growth opportunity, or simply the newest one.
If the practice is already sitting on $200,000 of opportunity in exam-only, contact lens capture, or treatment acceptance, improving those systems may generate the money that eventually pays for the new equipment anyway.
Optimize first. Expand second.
Put The Eyecare B.O.S.S. to Work in Your Practice
Are you reading The Eyecare B.O.S.S. yet?
Thousands of copies of the growth manual have already been sold, but the book is only the beginning. If you’re ready to start implementing the systems inside your own practice, visit EyecareBoss.com.
There are more than 40 free tools available to download, including the same tools used with practices to help turn the concepts in the book into execution.
And the results are worth paying attention to.
Across a year-long study involving approximately 25 practice locations, participating practices saw average:
Refractions increase 19.4%. Revenue increase 23.9%.
In a market where much of the industry is flat, those results stand out.
You can use the book and tools independently, work alongside other practice owners in a group, or get one-on-one coaching support. Your call.
